The village of Bobr said goodbye to Ales Pushkin. The political prisoner, a famous artist, died in the intensive care unit, diagnosed with a perforated ulcer.

Belarusian doctor Aliaksei Nosau told Charter97.org what kind of disease it is and how it can appear.

- If a patient died with a perforated ulcer, this doctor is either a complete wretch or just a real bastard, because you can't call him anything else. The fact is that a peptic ulcer is never quick, it usually develops slowly. Many factors influence it, especially bad diet, stress and so on.

At present, one of the bacteria, Helicobacter pylori, is considered to play a crucial role in the development of the disease. This bacterium is quite aggressive because it is one of the few that can survive in the acidic environment of the stomach. Most bacteria simply die there.

The disease develops with pain, of course. It can be of various kinds, for example, the so-called hunger pains that one feels before eating and after eating brings some relief. There are also satiety pains, which begin immediately after eating, due to the activity of acid in the stomach. The mere presence of such pains is one of the diagnostic factors that you have an ulcer, you don't even need certain tests for that. Then there is heaviness in the stomach after eating, constant heartburn, usually in the morning or at night. The reason I'm telling you this in such detail is because I had one myself. As you can see, I'm still alive.

Accordingly, it is not difficult to diagnose the ulcer at this stage, it is not even necessary to do research, but if you have done FGDS, fibrogastroduodenoscopy, you can clearly see it. The problem can be solved at this stage absolutely painlessly, without surgical intervention. One needs to follow a special diet. It is not allowed to eat fatty, fried, spicy and other aggressive products. It is also necessary to take antacids, which reduce the acidity of the stomach. If the ulcer is diagnosed, samples are taken and the bacteria are identified, then antibiotic therapy is added. And it is not necessarily to prescribe the strongest antibiotics, penicillin series does its best at the moment.

The next stage is perforation. One should completely ignore a person not to notice it. You can imagine a hole in the stomach through which the contents of the stomach get to the peritoneum. And the peritoneum is a covering that is responsible for the state of the internal space in the abdominal cavity. It contains a large number of nerve endings; the peritoneum reacts quickly and promptly to any aggression. And that is sharp dagger pain. Also, a perforated ulcer means bleeding because all the stomach walls are damaged and the person accordingly suffers from bloody vomiting and black feces.

These are reliable signs. At the initial stage, even organ-preserving surgery is possible, it is not necessary to do a stomach resection. It all depends on the size and condition. If there are major complications, if it is peptic ulcer, multiple ulcers, - stomach resection is done with a good result. Of course, a person's diet changes for the rest of his life, but he remains alive.

Then peritonitis develops. It is a purulent necrotic inflammation of the peritoneum. The pain is terrible. The peritoneum has a large number of nerve endings. If the operation is done and there is a good sanation of the purulent inflammatory foci in the peritoneum, roughly speaking, the abdominal cavity is well washed, then there is still a good chance of recovery.

Can you imagine how long it must take for a person to die in agony from peptic ulcer disease? Ales died in agony. It's terrible pain, unbearable pain. You can't think about anything. They're bastards, and my colleagues are bastards. It could have been treated painlessly in the early stages. Nobody lifted a finger.

- He was taken to intensive care unconscious. Is it possible to imagine that the prison did not notice that a prisoner was suffering from a perforated ulcer?

- Only someone who has never had an ulcer can say that, because the pain is unbearable. I repeat, stress, studies, emigration, many things happened to me, so many times I had these pains. I didn't even go for a diagnosis because I knew it was an ulcer, it can't be anything else, the symptoms are just obvious. By self-medicating and following a supportive diet, I recovered.

Diet, exercise and many other things prevent certain diseases from occurring. However, there have been times when I have faced these situations and healed. My answer is that it is impossible to miss a person suffering from a stomach ulcer, especially a perforated one. It's a crime, a serious crime. I just don't have the words.

- As a doctor, how would you describe the actions of the prison medical staff? Should they be punished?

- Absolutely. I understand that Lukashenka is a scumbag, as is his entourage, but the fact is that this crime is almost entirely in the hands of the staff. They are responsible for the treatment of a person. People who work there are accomplices of the regime; there are no other options.

I don't care what my colleagues think about me and so on. This is simply an unacceptable situation in medical ethics and medical practice. I can still understand if a person is somewhere far away, there is no contact, the symptoms are unclear or blurred, but in this case he is constantly in sight. One could probably see Ales writhing in pain. One could see him suffer. One could give him omeprazole in the morning before breakfast and he would feel better, plus a certain diet.
